I think there is nuance in the book, and the author's afterword makes her seem very sympathetic towards the main character. What Akari is doing is very unhealthy for her, but you can also understand to some degree why she does it. And while her family's frustrations with her are understandable, they also fail to support her in any meaningful way, perhaps thinking that the way they support her older sister should work perfectly fine for Akari, who obviously has different needs.
